Oestrogen Metabolism in Perimenopause: Why 'Putting the Bins Out' Matters

When we think about oestrogen during perimenopause and menopause, we often focus on how much we're producing. But there's another equally important piece of the puzzle – how well your body processes and clears it.


Oestrogen is one of the body's most important hormones. It influences much more than our menstrual cycle, affecting bone health, cardiovascular health, brain function, mood, cognition, skin, muscles and overall wellbeing.


During perimenopause, hormone levels fluctuate dramatically. You may have days when oestrogen is very high, followed by days when it drops sharply. These fluctuations, together with changes in how your body metabolises oestrogen, can contribute to symptoms such as:


  • Hot flushes and night sweats

  • Breast tenderness

  • Heavy or irregular periods

  • Migraines

  • Anxiety and low mood

  • Brain fog

  • Sleep disturbances

  • Weight gain around the middle


Having appropriate levels of oestrogen is protective and essential for long-term health, supporting bone strength, heart health, brain function, muscle mass, skin health and the health of the vaginal and urinary tissues. However, research suggests that inappropriate oestrogen exposure, whether due to excess production, poor metabolism or inefficient clearance, may increase the risk of hormone-responsive cancers, including breast, uterine, ovarian and thyroid cancers. It has also been associated with conditions including PMS, PMDD, PMOS, endometriosis and fibrocystic breasts.


The good news is that oestrogen metabolism isn't determined by hormones alone but is influenced by many factors including diet, alcohol intake, body fat, insulin levels, environmental chemicals, HRT, medications and your genetics. I like to explain oestrogen metabolism using the analogy of putting the bins out.

Phase 1: Fill the bins


Imagine you've finished a big house clean. All the rubbish has been collected into different bins, ready for collection.


This is what happens in Phase 1 of oestrogen metabolism, mainly in the liver. Your body breaks down oestrogen into different breakdown products (called metabolites), which are then prepared for removal from the body. Think of it as your body having three different routes for processing oestrogen: the 2-OH, 4-OH and 16-OH pathways. Ideally, we want to favour the 2-OH pathway, keep the potentially harmful 4-OH pathway well controlled, and maintain a healthy balance of the biologically active 16-OH pathway. Whilst the 16-OH pathway has important roles in maintaining bone density and supporting vaginal and urinary health, it may increase oestrogen's effects in certain tissues such as the breast and uterus, which isn't always desirable.


If phase 1 isn't working efficiently, or the balance between your 2-OH, 4-OH and 16-OH pathways isn't optimal, it may contribute to ongoing hormone symptoms during perimenopause. This is where a DUTCH test can provide valuable information by assessing the balance of your oestrogen pathways.


Phase 2: Put the bins out


The bins are packed and ready, but they still need to be taken outside so they can be collected.


In Phase 2, your liver 'packages up' these oestrogen breakdown products through processes known as methylation and glucuronidation, making them safe to remove from the body. This stage relies on good nutritional status. Nutrients including folate, vitamin B12, vitamin B6, magnesium, choline and glycine all help these pathways work efficiently. If this process is sluggish, hormone metabolites may not be cleared as effectively as they should be.


Phase 3: Rubbish collection


Once the bins are out, the rubbish lorry needs to collect them and take them away for good. This final phase happens largely through the bowel as waste oestrogens are excreted in the stool.


An important player here is something called the oestrobolome – the collection of gut bacteria that helps regulate how much oestrogen is eliminated from the body. A healthy, diverse oestrobolome supports the efficient removal of oestrogen, whereas an imbalanced gut microbiome may reduce how effectively this happens.


If you're constipated, your gut microbiome is out of balance, or certain gut bacteria produce high amounts of an enzyme called beta-glucuronidase, those packaged hormones can be 'unwrapped' and reabsorbed back into the bloodstream. It's a bit like carefully putting your bins out, only for someone to bring them back into the house before the rubbish lorry arrives! By supporting your gut microbiome and oestrobolome, you can help improve the body's ability to eliminate excess oestrogen efficiently.

Although we can't stop the hormonal changes of perimenopause, we can support the body's ability to process and clear oestrogen efficiently.


Some of the most effective strategies include:


  • Eating plenty of colourful vegetables, especially cruciferous vegetables such as broccoli, kale, cabbage and Brussels sprouts.

  • Eating adequate protein to support liver detoxification pathways.

  • Ensuring good intakes of nutrients involved in methylation, including folate, vitamin B12, vitamin B6, magnesium, choline and glycine.

  • Eating plenty of fibre to support regular bowel movements and feed beneficial gut bacteria.

  • Looking after your gut microbiome.

  • Staying well hydrated.

  • Limiting alcohol where possible.

  • Supporting healthy blood sugar balance by eating balanced meals and staying active.

  • Reducing exposure to hormone-disrupting chemicals, such as some plastics, fragrances and pesticides.
